Preparing for Your Clinic Visit

PATIENT FORMS TO COMPLETE PRIOR TO YOUR FIRST VISIT

Patients new to Silverman Ankle & Foot should download, print out and complete each of the following forms. Please send the completed forms by mail to our Edina office or fax us at 952-224-8515 at least one week prior to your visit. We will confirm receipt of the forms and the appointment by your preference of email or phone.

If we have not received your forms in sufficient time prior to your appointment, you will need to complete them in the office. And, as unexpected things can happen, we suggest bringing a copy of the completed forms with you the day of your appointment.

SHARE YOUR PRIOR MEDICAL RECORDS AND IMAGING FILMS

If you have been evaluated or treated for this problem by another health care provider, please call their office and request a copy of your medical records and films. Although, it is more convenient to have them sent to us, it is more reliable if you pick them up and bring them to your appointment.
